What is Tirzepatide?

Tirzepatide is a synthetic peptide consisting of 39 amino acids, belonging to the class of glucose-dependent insulinotropic polypeptide (GIP) and glucagon-like peptide-1 (GLP-1) receptor agonists. This innovative compound was developed by Eli Lilly and Company and represents a breakthrough in dual incretin receptor agonist technology. Its molecular formula is C225H348N48O68 with a molecular weight of approximately 4813 Da. Structurally, Tirzepatide is based on the natural GIP hormone, but contains specific amino acid modifications and a C20 fatty acid chain covalently bound via a linker, which significantly extends its half-life.

Mechanism of Action

Tirzepatide functions as a dual agonist that activates both GLP-1 and GIP receptors, resulting in a synergistic effect on glucose homeostasis and energy metabolism. Upon binding to GLP-1 receptors, it stimulates glucose-dependent insulin secretion from pancreatic beta cells and suppresses glucagon secretion from alpha cells. Concurrently, binding to GIP receptors activates additional insulinotropic effects and promotes glucose uptake in peripheral tissues. The compound activates adenylyl cyclase, leading to increased cAMP levels and activation of protein kinase A (PKA). This cascade results in phosphorylation of CREB and other transcription factors involved in glucose metabolism. Additionally, Tirzepatide slows gastric emptying and influences satiety signals via hypothalamic pathways.

Research Results

Preclinical studies have shown promising results for Tirzepatide in various animal models. In diabetic db/db mice, a 4-week treatment with Tirzepatide led to a 2.1-2.8% reduction in HbA1c levels compared to control groups. Research in obese Zucker rats demonstrated a dose-dependent weight reduction of 15-25% after 8 weeks of treatment, accompanied by improved insulin sensitivity. In vitro studies with INS-1 pancreatic beta cells showed a 3.2-fold increase in glucose-stimulated insulin secretion at physiological glucose concentrations. Long-term studies in non-human primates showed sustained effects on weight regulation and glycemic control for 20 weeks, with an average weight reduction of 18.7%. Pharmacodynamic studies confirmed dual receptor activity with EC50 values of 0.06 nM for GIP receptors and 2.39 nM for GLP-1 receptors.
